TATTOO DEPOSIT & CONSENT FORM

DEPOSIT AGREEMENT

Deposit Amount

TERMS & CONDITIONS 

  1. Non-Refundable Policy 

    The deposit secures your appointment date, time, and custom design work. This payment is strictly non-refundable under any circumstances. 


  2. Deduction From Total Price 

    Your deposit remains active throughout your entire tattoo process. The full deposit amount will be deducted from your final tattoo session price. 


  3. Rescheduling Rules 

    You may reschedule your appointment up to three (3) times without penalty. You must provide at least 48 hours' notice before your scheduled time. 


  4. Cancellation & No-Show Policy 


  1. Late rescheduling under 48 hours forfeits your entire deposit. Failing to show up to your appointment forfeits your entire deposit. Exceeding three (3) rescheduled appointments forfeits your entire deposit. 


  2. Policy Enforcement 


    Forfeiting your deposit immediately terminates your current booking. Art by Bones will require a brand-new deposit to restart your process.
